Transoesophageal echocardiogram. Upon closer interrogation at various windows including 0°, 125°, 46° and 112° angles, it was noted that the apparent inferior vena cava ‘arm’ was indeed the Eustachian valve and that the inter-atrial septum was in fact, intact. The turbulent jet seen flowing from right to left was determined to be the severe tricuspid regurgitation jet, directed towards the inter-atrial septum during systole.
